Enroll in the new Anthem vision and dental plans.

Follow these simple steps to enroll in your new vision and dental plans.

(Please note: The instructions below are set up for individual enrollment only. If you are interested in setting up an employer plan, please contact Mark Williams at 860-351-0107.) 

1. Call Lauren Kuhn at Progressive Benefit Solutions at (203) 208-4841 to get set up in the system. Please note the following information will be required to get enrolled:

• Social Security Number
• Name
• Date of Birth
• Gender
• State of Residence
• Email address

2. Once you are set up in the enrollment system, an email will be sent out with a link to enroll in the benefits. (View instructions of how to enroll.)

3. Please note: Once the confirmation page has been signed electronically, in order to process enrollment, you must complete the banking form in the Task section and attach to the enrollment site. This will allow the monthly premiums to be deducted from the account around the first of every month for the Dental and Vision premiums.
